投標人需提供以下文件

*（1）投標人須按招標文件第一冊第四章格式IV-9.資格證明文件格式的要求填寫和提交有關資料。
*（2）投標產品原產地在關境外的投標人必須是制造商；或者是制造商直接授權的代理商（進口產品開標前未進入中國關境內的，投標人必須是在關境外注冊的公司；進口產品開標前已進入中國關境內的，投標人必須是在關境內注冊的公司）。投標人如為制造商，須提供生產廠房證明材料及生產設備明細表；同一品牌的制造商和代理商不能同時參加本項目的投標。若為進口品牌，投標人投標的產品必須為設備生產廠家官網可查的原裝進口產品，投標人需提供設備原產地證明、報關單。投標人為代理商投標需具備以下條件：
①必須提供制造商對本次投標的唯一正式授權書（授權書必須由制造商直接授權，不允許二級授權）；授權書中應包括″制造商對產品及售后服務質量負責”的相關內容和制造商聯系人及聯系方式，并確保制造商聯系人能夠協調處理項目執行過程中的重要事項。（對于此要求，制造商也可在授權書外另行出具書面承諾/說明），否則不接受代理投標；
②投標人在當地依法登記注冊的時間距開標日滿三年，需提供營業執照等注冊登記的有效證件復印件；
③距開標日三年內，投標人須具有同類產品至少兩個不同年份的銷售業績，需提供證明其銷售業績的合同或訂單復印件（至少提供2份不同年度的合同或訂單復印件）或用戶證明，并由投標人代表簽字或蓋章。
*（3）投標產品原產地在關境內的投標人必須是制造商（不接受代理商投標）。
*（4）投標人未被中國石油招標投標網(www.cnpcbidding.com)暫停或取消投標資格。（根據《中國石油天然氣集團有限公司投標人失信行為管理辦法》的規定，失信積分達到8分時，暫停失信行為人投標資格半年；失信積分達到9分時，暫停失信行為人投標資格一年；失信積分達到10分時，暫停失信行為人投標資格兩年。暫停期滿后恢復投標資格，有效期內的失信記分繼續執行。失信積分超過10分或在暫停期間再次發生失信行為的，取消投標資格三年。）上述內容以評標委員會現場在對應網站的查詢結果為準。
*（5）投標人未被納入中石油集團公司供應商、承包商、服務商黑名單（集團公司供應商、承包商、服務商黑名單可在中國石油招標投標網/www.cnpcbidding.com查詢）。上述內容以評標委員會現場在對應網站的查詢結果為準。
*（6）距開標之日前一年內，投標人在中國石油物資有限公司組織的招標活動中對評標結果的無效投訴（包括中石油集團公司機電辦處理結果為：不予受理、駁回、維持原評標結果）達到一次及以上的，投標將被否決。
*（7）投標人須提供開戶銀行在開標之日前三個月內開具的資信證明原件或該原件的復印件（由投標人代表簽字或蓋章）。
*（8）第八章中要求的其他合格的資格證書。
*（9）投標產品的業績要求：
2020年1月至投標截止日，投標人須提供5份及以上自動密度測定儀在煉油化工裝置應用的銷售業績。業績證明材料：提供合同原件和技術協議原件掃描件；提供合同用戶聯系人及聯系電話。
(1) All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
(2) The bidder whose product origin is outside the customs must be the manufacturer; Or the agent directly authorized by the manufacturer. The agent shall meet the following conditions for bidding:
① The bidder should provide a letter with original sole official authorized by Manufacturer for the bid；
② the Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
③ The bidder shall have sales performance for similar products in two different years within three years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;
(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) (6) The bid shall be rejected if the bidder puts forward invalid complaint one or more times in the bidding activities organized by China Petroleum Materials Co., Ltd. within one year before the bid opening date (including complaint rejected, overruled or judged to maintain the original bid evaluation results by the mechanical and electrical office).
(7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.
